What it means

The operation with this key was processed, but its response could not be recorded or can no longer be decrypted. Nothing runs twice (§5).

What to do

Do not retry with the same key. Read the current state with a GET, and use a new key only for a genuinely new operation (§12.4).

Branch on code, never on title or detail. detail varies; quote request_id to support.
